Job Summary
Responds to customer needs for clinical information in both paper and electronic media. Assures that the “clinical user” receives appropriate clinical information for the specific use intended: Patient Care, Quality Reviews, follow-up, research, and teaching.
Job Duties
- Demonstrates the WSMC Customer Service Values, which are key in providing quality service to patients and customers.
- Determines with interactions with physicians, which data from previous admission or encounters is necessary for current admission, and /or encounter
- Provides data as needed for patient care, follow-up research and other uses such as third party reviews either through retrieval of data from various systems upon request of user staff or compiling, preparing, or mailing special requests for information.
- Identifies documentation types, indexes and provides assembly of records in both electronic and paper formats
- Enters patient data in a timely manner to assure that a Complete Data Set are available and accessible to users
- Works with users to ensure all pertinent information is included and available for their use
- Assures that clinical data is returned from its intended use, and made complete with the addition of new material of data.
- Reviews records to determine and assure that data retrieved (electronically or in paper mode) meets the requirements of the user/requester
- Reviews various reports and selects pertinent information; corrects and verifies information to ensure accuracy; understands the structure and content of the medical record
- Performs final review of the record at the time of filing (or electronically) to assure accuracy of data and monitors missing components
- Continually enhances skill sets through continuing education in clinical documentation and assigned clinical department.
Job Qualifications
- High school diploma
- One year clerical experience in Hospital Medical Records or comparable clerical setting
- Strong understanding of the structure and content of medical records, which can be obtained through 6-8 weeks of on the job training.
- Basic knowledge of medical terminology strongly preferred
- Good PC skills, data entry, database systems
- Strong written and oral communication skills
- Strong interpersonal skills and customer service
