JOB SUMMARY

Under the general supervision of the Clinical Nutrition Manager, provides optimal nutritional care for all patients.  Develops, implements, monitors and evaluates nutritional treatment plans in collaboration with the multidisciplinary health care team.  Documents nutritional services provided.  Participates as a health care team member and supervises the activities of support personnel.  

JOB QUALIFICATIONS

Must possess current licensure and registration as a Licensed Dietitian and Registered Dietitian in the State of Illinois or be RD eligible.

JOB DUTIES

  • Assesses complex patients’ nutritional status based on physical, laboratory, dietary, anthropometric data, review of the medical record and appropriately following ADA’s evidenced based practice guidelines. The Clinical Dietitian will develop and implement a plan of nutrition care to meet assessed needs that is patient-focused and is correlated with other immediate and long term health care goals. 
  • Determines outcome indicators that are relevant to the patient’s needs, nutrition diagnosis, nutrition goals and disease state and condition. Monitors patient progress, measuring and evaluating progress following established charting and documenting procedures until discharge. Identifies changes in outcome indicators such as weight, feeding ability and redefines goals/outcomes as part of patient monitoring.
  • Collaborates with physicians and other members of the health care team to provide quality patient care.  Accurately identifies and notifies team and physician of necessary modifications to food, nutrient intake and route of administration.  Acts as an informational/educational resource to nursing staff, medical personnel, patients and their families.  Assists with the development, review and revision of patient education materials.
  • Communicates and documents patients’ nutritional care plan, nutritional progress, and diet instruction information in the medical record, according to department and hospital standards.
  • Serves as clinical nutrition expert and assumes responsibility for assigned patient care floors or specialty area.  Attends scheduled floor meetings as assigned and participates in health care team rounds/conferences.  Shares knowledge and expertise; provides leadership and direction to diet technicians/dietitian assistants and acts as a resource to other staff members.   Participates in/provides education presentations/in-services for department employees as well as other members of the health care team.  Participates in community outreach initiatives as assigned.
  • Participates in Performance Improvement and peer review activities as assigned.
  • May rotate responsibility between inpatient and outpatient duties to ensure cross training in all areas/specialties and to provide for the most effective patient care.
