JOB QUALIFICATIONS
- Level of knowledge acquired through the completion of a Master's degree in Pharmacy or Pharm. D.
- Completion of a hospital pharmacy residency or two (2) to three (3) years’ experience as a registered pharmacist in a hospital setting.
- Current license by the State of Illinois as a Registered Pharmacist.
- The interpersonal and communication skills necessary to work effectively with patients, physicians, and other hospital personnel.
- The demonstrated clinical knowledge of drugs and disease.
- Antimicrobial Stewardship experience preferred
JOB DUTIES
- Those served include recipients and/or the family/caregivers of recipients of chemotherapy for cancer and its complications, as well as providers and nursing staff
- The oncology pharmacist provides education regarding the safe and effective use of chemotherapy, both intravenous and oral, and associated medications to the healthcare team, patients and families
- The oncology pharmacist facilitates the processing of chemotherapy and other orders for patients served in Cancer Center Infusion
- The oncology pharmacist identifies, solves and prevents medication-related problems in the hematology/oncology population
- The oncology pharmacist assists department leadership with chemotherapy-related regulatory compliance and financial considerations
- The oncology pharmacist, with appropriate competencies completed and maintained, may perform pharmacist duties related to chemotherapy dispensing or other staff pharmacist duties of the Pharmacy Department
- Act as the pharmacy liaison to the Hematology/Oncology Clinic and Cancer Center Infusion
- Available to provider and nursing staff in both the Hematology/Oncology Clinic and Cancer Center Infusion for questions on medications including both oral and/or intravenous chemotherapy and supportive care medications
- Provide electronic health record treatment plan expertise and support to the Cancer Center team
- Provide individual patient/family counseling on chemotherapy regimens, adherence to chemotherapy and/or supportive care medication regimens and adverse effects of chemotherapy
- Provides medication order verification and clinical services to the nursing staff, providers, and patients
- Work includes staffing our outpatient retail pharmacy, located in our infusion center
- Work includes providing direct supervision of interns, technicians, and student work activities
- These include dispensing; sterile products preparation; inventory control; repackaging and providing the professional check for these processes
- Responsible for the control, dispensing, and maintenance of all medication used within the hospital
- Is able to perform or assist in departmental tasks that are associated with these procedures
- Participates in departmental or systemwide programs that work towards quality assurance and performance improvement
- Participates in regulatory compliance
- Collects data and evaluates medication events, adverse drug reactions, quality assurance and drug utilization review
- Demonstrates the ability to access, enter and adjust patient information in the electronic health record
- Other duties as assigned
Pay Rate: $52-$57 / hour
